Okay, yesterday we were talking about gastric bypasses and dumping syndrome. Two good cups of coffee and I was reminded of the conversation … don’t ask me why, this is a family blog.
Anyway, one of our surgeons at my place was a year behind me in med school and ended up practicing here too. She is a general surgeon and began a niche practice of laparoscopic gastric bypass surgery. There’s a lot that goes into it (some would say “a ton”!!!) from preoperative nutrition counseling and special surgical equipment and techniques. When she was really getting going with the surgeries I started to see these big boy hospital beds showing up in the hospital.

The title of the article at the site reminded me of my early days as a student and intern where occasionally we would have to transport an inpatient from the hospital to the nearby Tufts Veterinary School to use their large animal scanner for a CT on an obese patient. They eventually built models that could handle the fatties
We had to order one for a patient who was pretty big. Ordinarily you get the help of another staff in order to boost a patient in the bed when they’re too far away from the pillow. Not possible or safe for your back, with a patient that is very large. With this thing, you press a couple of buttons and the sheet, which is very long and wraps around a spindle behind the head of the bed, up like a conveyor belt to boost the patient. Pretty cool.
I see a couple of you cats had insomnia this morning too. I gave up trying and showered at 5:30.
I’m in that pattern where you finally get back to sleep in the morning and then oversleep. Can’t keep doing that.
Of course, now that I’ve taken a sip of coffee, I feel like I could go back to bed pretty successfully. But, resist we much.
I’m taking one of those courses I told you guys about at edx.org. Just auditing so I don’t have to do the homework or labs. Still pretty fun and interesting.

Another amusing $20 weight loss story.
I bought a half duplex when I was an intern and had my brother and a med student for roommates. The med student was a piece of work. He had lost a lot of weight on a strict diet and now had a lot of excess skin folds to deal with. He worked out a deal with the hospital for the plastic surgery fellow to excise the abdominal flaps and eventually had it done. This was in the days of generous narcotic prescriptions after surgery and he took Percocet like candy for days. At some point he had to take a crap and of course he was all bunged up like he was trying to pass a pineapple. Thankfully I was spending most of my life in the hospital those days. My brother told me he was in the bathroom on the shitter for hours moaning and talking to himself. Within a couple of weeks he had a wound dehiscence which is when the closed incision opens up and drains. Because of all the fat they had to excise he formed a seroma despite placing drains that he kept in for days after the procedure. He left his nasty fluid soaked gauze pads piled up in the bathroom trash … like a small mountain of them. After asking him for days to clean them up and getting nowhere I brought gloves home from work and piled them on his bed. After he recovered from all this I came home from work one day to see him studying at the living room coffee table with a plate of cheese covered nachos, 2 liter bottle of Mountain Dew (no glass needed, it was a personal bottle to chug on) and a pack of giant Pixy sticks. He got the boot soon after because he found a girlfriend who ended up living at my house and staying in his room only exiting when I and my brother were away. I’m guessing these days he’s got the enormous gut again and the bonus surgical scars. I say “guessing” because we didn’t keep in touch as you might imagine.

I’ve been waking up pretty early. I actually wake up all night – each time praying that its a decent time and I can get up. First time last night? 12:30. I just repeat this pattern all night until its at least after 5:30. After 5 though usually just remain in bed, awake, for the duration.

I saw this firsthand and even predicted it with a coworker who seems to always collect interesting diagnoses. In her case, it’s a plea for attention.
But the constellation of symptoms is eerily similar to the ones that depressed people report. I’d bet anything that most of these people actually have an underlying depression causing them to experience these symptoms.

This is really interesting. We’ve kind of made up all sorts of illnesses for different people. A lot of it seems to be slight anxiety about something which most people call LIFE. The entire generation below me is basically a hodgepodge of invented disorders.

Now, I don’t really know why they would be a particular target – and I can only guess as to why there is no media attention, if it is a real thing.
But, you know how these things are. They bring up a number and it seems outrageous, but 1) I don’t know if the number is correct or 2) out of proportion to any other demographic.
I mean, I’m going to speculate … because it’s what I do. I looked into a few of the missing stories and it always involved shady men and drugs, and leaving with people who she said were “friends” of her’s but no one really knew them.

See, my issue here is that they want to compare the Gabby case to other cases which are a bit different. Black prostitute or drug addict goes missing? It just doesn’t have the same human interest as Gabby – who could have been anyone’s straight shooting child. People need to stop comparing things that aren’t alike and pretending like it means something.
When a girl leaves with some drug dealers, and never shows back up again – well, really bad choices, but not as big of a mystery. Or as compelling. It just isn’t . Bad things happen to stupid people.

They don’t talk about those cases because the perpetrators are likely to be minorities and the stories raise questions about what the hell goes on on the reservations. Ditto why you don’t hear discussions about the abuse of poor black kids in the ghetto by mom’s live-in boyfriend or the kid’s stepdad, or by foster families: it raises uncomfortable questions.
I think there’s a legitimate criticism of the media for ignoring those stories precisely because they lead to larger discussions about the way that politicians, bureaucrats, and self-appointed “leaders” have failed those communities. Those are stories that decent, honorable journalists should want to pursue.

I heard that Daniel Horowitz sent a 45 min video to Deace about an older gentleman that won a court case initially to have his wife treated with ivermectin in a hospital. They couldn’t give it to her, because of hospital policy. Court case allowed it, and she improved. But then the decision was reversed, and the hospital pulled the ivermectin. She died.
I consider that pretty much murder. I don’t see how they could rule it otherwise.

Back in the early eighties the architectural firm I worked for was hired to redesign the thoracic surgery offices at UofM Med. Center. These were the offices where doctors examined patients, not the actual hospital where they treated them.
When they first rolled out the existing plans, I noticed that inside the offices were these huge closets. When I asked what the closets were for they explained that one day a week (Thursday) is “fat” day. All of the waiting room furniture gets swapped out for huge chairs needed for the fat people. These were special built enormous chairs. They had learned long ago that when the fat people sit in the normal chairs they either couldn’t fit, or they crushed them.
So all the fat people were scheduled for Thursdays – the custodial staff swapped out the furniture each Wednesday night.
I said, “Why don’t we just design custom benches that would fit, let’s say four normal people, and on Thursdays one fat person could use it, then we could capture the space that the closets occupied, and make both the waiting room and the offices a bit bigger.

The hardest part of living for a lot of doctors is realizing that extensive knowledge of one facet of life doesn’t extend to all areas. That’s why doctors typically make for lousy airplane pilots and investors.
So my first problem with the MMIP panic is the acronym. It started as MMIW (missing and murdered women) but that wasn’t inclusive enough, so it expanded to MMIWG (missing and murdered women and girls), and that still want inclusive enough, so it’s the letter P for people now.
You might think that’s an unimportant quibble, out just general eyerolling at the never ending mission creep of the alphabet people. But there’s more to it than that.
I watched a Senate hearing a while back, in which Jon Tester wanted some government type to acknowledge that the MMIP crisis was a crisis. But if it’s a crisis, and we have to Do Something* about it, then we have to understand the nature of the crisis, yes? But the problem affecting indigenous women is not necessary the same thing as the problem affecting indigenous girls or indigenous men. So there’s no one size fits all solution.
Same deal with having to M’s in there. The crisis of murdered women is not necessarily the same thing as the crisis of missing women. Sometimes a woman is missing because she was murdered and the body hasn’t been found yet, but sometimes that’s not the case. And again, if you want to come up with a solution, you need to understand the problem, which starts with not conflating many different problems.
Congress passed a law called Savannah’s Act, which mandates certain data collection requirements. It’s named after Savannah Greywind, a pregnant woman who was murdered by a crazy last who wanted to steal her baby. Fargo Police did a great investigation, caught the perpetrators, and convicted them at trial. Happy ending, right? Well yes, as much as the justice system can provide. There’s nothing in Savannah’s Act that would have saved Savannah’s life, or anyone else’s life.
In Montana, we have Hannah’s Act, which mandates certain reporting requirements within strict time limits when someone goes missing. It is named after Hannah Harris, who was murdered by a couple and hidden in the woods behind their house. She was already dead before the time limits specified in Hannah’s Act would have kicked in, so Hannah would not have been saved by Hannah’s Act. Also, the Act has these time limits for reporting, but never says what happens if the reporting is delayed. Can the cops be sued? Fired? Thrown in jail? Defunded? These seem like important questions that the legislature didn’t bother answering.
Call me old fashioned, but I think if you’re going to pass a law to prevent a certain mischief, then it should reasonably be expected to prevent that mischief before you pass it.

So the next issue is, who is perpetrating these crimes? In my experience, overwhelmingly, natives kill other natives. Just like white on white or black on black. It’s got little to do with race, and a lot to do with who you spend your time with, and if someone lives on a reservation, the people they are most likely to come into conflict with are other people who live on a reservation.
And yet, there are a lot of people who think the real crime is white people coming on the reservation for human trafficking purposes. I can’t say that doesn’t happen, but I’ve literally never seen it. Most often it’s a domestic violence situation, or an argument, where no one intended to kill the other person, just rough em up a bit, and they went too far. That’s not every case, not by any means. But it’s a lot of them.

As for media coverage, I have no idea why national news loves the missing white girl so much, but I do know natives don’t usually get a lot of national coverage. I suspect it’s for the same reason that none of us know the names of anyone who was murdered in Chicago last week.
I had a trial in one of my cases where one AP reporter showed up, in addition to local media. Usually there is no media, and rarely we’ll get a local reporter, but that’s it. The local media doesn’t have enough resources to have a dedicated crime reporter.

In the case of a missing person, regardless of race, national media coverage usually isn’t helpful. You want local coverage so that local people can keep their eyes open or join search efforts.

NOW Foods makes a good D3 supplement at 5000 IU. A gram of vitamin C, a decent quercetin, and possibly a baby aspirin or two. That latter is for a tiny bit of blood thinning to combat clots caused by spike proteins, whether from virus or so-called vaccines.
Probably also want a magnesium citrate supp, but that’s just because almost no one gets enough from diet.

Zinc gluconate is poorly absorbed, it’s best for sunscreen. Glycinate is supposedly mu better taken up.
I have no thoughts on K2+D3, Roamy. K2 is easy to get from meat and good cheese, so I’ve never bothered trying to supplement it.

The book came out in ’62, the movie in ’75. The de-institutionalization movement was in full-force by the late 60’s. 12 Monkeys came out in ’96 so I don’t think that was an influence unless there was some more policy reforms that came later, which could well be.
You probably know about Dr. Al Danenberg from Mark’s Daily Apple. He suggested vitamin K2 for calcium metabolism and preventing artherosclerosis, so I am taking 45 mcg K2 and 400 IU D3 daily. If nothing else, I have a lot less tartar buildup on my teeth.

Interesting. Whenever I did a week of dietary tracking I always came out comfy on K, E, selenium, and low on magnesium and not always great on C. I knew I needed D3, so I added Mg and C if I didn’t have any citrus that week.
Rich animal sources include high-fat dairy products from grass-fed cows, egg yolks, as well as liver and other organ meats (30).
Egg production in my house tracks with consumption, so I haven’t had as many lately as the days have shortened, but I’ll be noshing almost exclusively on grass-fed lamb soon.
I was wrong, the 400 IU D3 is in my multi-vitamin, the additional supplement is 2,000 IU.

K and K2 are not the same thing. I thought you could only get K2 through certain fermented foods. I was taking K2 for some time, but ran out and didn’t really notice much of a difference. FMMV

We actually get to share the results of continued development, which I’ve heard allows us to sidestep some missile defense restrictions in various treaties. Plus, Iron Dome gives the Israelis more flexibility with regards to their responses, which in turn gives others in the region a bit more diplomatic breathing room.

I eliminated a whopper of an upper respiratory infection 1 1/2 years ago by eating very sour kimchi and drinking the kimchi juice. The improvement in how I felt after consuming the kimchi was stark. I assumed it was the the lactic acid and competitive beneficial bacteria knocking out the infection in my throat, but maybe it was K2.
As I understand it, most people make K2 from K1 just fine, and it’s a demand-driven process. Excess K2 as K2 can be fatal, just like the animal form of vitamin A (which is why grizzly bears are poisonous).

Fall hit here. The grass and weeds went from green to brown last week, and this morning it was 43 degrees. Growing season is over, but we had a pretty good year, at least enough grass to make it to next year.
Looks like we need common sense bow regulations.
Ok, I didn’t know that on the K2. So I went back and did some digging apparently there are multiple forms of K2. Organic chemistry does not do well with shorthand and vitamin nomenclature is screwy. The MK4 is in eggs butter and meat. The MK7 is in Natto, Sauerkraut and hard cheeses. But hell if I know, I’m just repeating like a parrot crap off the internet.

I spent one year in Denver for my fellowship and it was the first year of my life I was not in New England for the fall season. The only thing I noticed was some aspen trees turning yellow and losing their leaves. It was only the next spring when I noticed the hills turning green when the grass grew back. Definitely a subtle shift if you’re not used to it.
We’ve had so much rain this summer that the leaves haven’t started turning yet. Usually a lot of the maples are turning red in late August and certainly by late September.
