    I’ve spent a ton of time in this area. Antioch and Kenosha are about a 25 min drive apart, if that. It’ll present legal issues that he’s 17 and went to WI with a gun, though.

    He was running away, the mob pursued and threw objects, and once he fell they were on him attacking him. That would seem to fit under the latter part of section (a) as well as section (b).

    Why was he there and alone? Who said “Son, we’ll put you here by yourself and if anything happens you tell them to stop”? How does a minor have possession of a long gun without an adult supervising him?

    I’m wondering how a rifle can shoot an arm off. I mean, it can happen, especially at close range by disrupting blood flow to the hand and causing destruction of the vessels and nerves to the arm or trashing the elbow joint to the point where it can’t be reconstructed and you’re better off with an amputation but to know definitively at this point that he blew his arm clean off is weird. If it’s been confirmed I haven’t seen that yet.

    .30 rifle at close range can be a lot of tissue damage, especially if you had a tangential strike on something like an upper arm bone. Plus the bicep is attached at multiple points on both ends, but if those are sheared it’ll curl up like a rubber band snapping.

    Bullet probably didn’t blow his arm off. With higher velocity bullets, you get hydrostatic shock which causes a lot of damage, plus if the bone was hit and shattered, there would be tissue damage from the bone splinters. The one pic I saw just looked like a hole through the muscle. Arm still attached.

    My sister works at Trader Joe’s
    Lady comes in and gives them a box of masks she made. The gesture was nice and all, but like, no one is interested in your mask.

    Trader Joe’s is only letting in like 30 people at a time to maintain “social distancing” so there’s always a line outside the door.
    Lady shows up yesterday and walks to the front of the line and is like, I’d like to be let in.
    Dude at door is like, ok, that’s cool and everything, but there’s a line.
    She says, but I’m the lady that made you all those masks.
    Dude is like, that’s very nice of you, but it wouldn’t be fair to all these people waiting in line.
    She goes to the back of the line, but is pissed and stomping her feet and clearly expressing her irritation.
    She finally gets in the store, walks up to the manager (they call them Captains) and says she wants all the masks back she gave everyone.
    Captain is like, ooook. Gets the box and gives it back to her.
    He says, is there other shopping you’d like to do?
    Well, I was going to, but I’m never shopping here again.
    She paces back and forth a bit…and then starts handing out the masks to employees again.
    They’re like nah, we’re good. You need to leave the store

    I didn’t find $20 because I wasn’t there, nor did I get a hand made mask by a crazy person

    I don’t have a lot of experience treating GSW, so called penetrating trauma. My training was mostly with blunt trauma from MVAs. The few cases I did see had tissue damage out of proportion to the fracture seen on film.

    The one case that stands out in my mind was when I was in Denver. I did moonlighting covering the old Denver General for night call. The residents did all the work and I was the attending of record for billing. One night they called me to help out on a GSW in a 20 something guy who was running from the cops and got shot in the knee. It was like a jigsaw puzzle but the leg was viable. After 3 hours of them fucking around trying to put it together I called it and said no mas. We put a spanning external fixator from thigh to leg to hold it out to length. That’s when the anesthesiologists said his forearms were tense. Sure enough he had compartment syndrome of both forearms. The residents were psyched that they were now able to do fasciotomies of the forearms on the poor bastard.

    Compartment syndrome happens when the fascial compartment pressure is high from either a fracture or fluid overload and the arteries can pump blood in but because the venous occlusion pressure is high the blood can’t return. Most common in the leg with a tibia fracture but it can happen in the forearm or thigh. We’re taught to look for “The 5 P’s”.

    The big one is pain out of proportion to what is expected. When the nurse is calling you because the pain meds you ordered aren’t working and your patient is screaming in pain it’s time to consider compartment syndrome. It’s not super common but can be devastating if missed. A fasciotomy is when you go in and open up all the fascial compartments (4 in the leg, 2 in the forearm).

    A common fracture in kids is a supracondylar elbow fracture and we treat them with CRPP (closed reduction percutaneous pinning). That is what we do now with live fluoroscopy to place the K-wires but before they had that they used to hyperflex the elbow to hold the fracture in place. Which led to a lot of problems with the blood flow due to swelling.

    For some dementia or Alzheimer’s patients the part of the brain that regulates sleep could be damaged (holes in it). It’s amazing but that means they could be up all day and you assume they’ll sleep like a baby but they go all night too (I was going to say, like your mom but I won’t).

    The regulator just isn’t functioning and they will need meds to sleep or to relieve anxiety just to let their bodies rest.

    Yeah, totally right. If they win, well then, they were right all along.

    If they lose they get to do 2016-20 over again pretending he’s not really the president.

    I’ve been tracking the toss up state polling balanced with the 2016 polling error for about the past 45 days and there’s a lot of movement to Trump. He’s polling ahead of where he was in 2016 and I expect that he’ll close strongly again.

    Biden is about 15-30 days behind the news cycle, I’m assuming because he’s surrounded by establishment types that have no idea how to get real information. Because of that he’ll make poor decisions that are always too little too late. McCain and Romney come to mind.

    Blood squirts out of the fracture site but the amazing thing is how the muscle pops up as the fascia is released. You can’t close the skin afterwards or it will happen again. After a couple more trips to the OR you can usually close it up. Wound VAC therapy helps close it up too. Or a skin graft.

    I’ve got lots of respect for you all who do that work. While I can watch some surgeries being done, I wouldn’t have the stomach to do it myself. Pulling a tick out of one of my kids gave me the willies. Looking at a throat freshly removed of it’s tonsils made me lightheaded. Reading my pathology report made me feel queasy in my swimsuit region.

    PJM, it is rumbly and rainy here. I put off grocery shopping until tomorrow, which I’m sure I will regret.

    I’m sure someone else has already said this, but it was one thing when the BLM protests were burning the big cities. Chicago, Portland, Seattle, NYC have been shithole dumps for some time. But Kenosha is 100K people, and now people are thinking, whoa, this could happen in my town.

    First cutting of my own private hayfield yielded 200 bales at a rough cost of $2.50 per bale. Going rate for a delivered square bale is $7.50, so we saved about a grand overall. I should have hired some teenagers to stack it, but it worked out. Might just use the field as pasture going forward, though, coordinating with the guys who have hay equipment was a nightmare.

The one with a big black thing in the bend of the elbow (antecubital fossa for Lauraw) is a wound vac sponge. You can see a tube off to the side applying the vacuum. Tough place to be shot, there’s a lot of valuable real estate RIGHT THERE!

    4 acres, grass hay (no alfalfa, no clover). For reference, that’s enough hay to feed 2 full-sized horses and 2 miniature equines for 200 days. It’s also just about enough to take two weaned steers from purchase to market weight, iirc. I could be meat independent and earn a (meager) income on just that hay production, and that’s just one cutting in year 1. 2-3 cuttings is normal.
